CareNet, Inc. (hereinafter referred to as “CareNet”) recognizes that it is important to securely manage the personal information of employees of CareNet and also the personal information of its customers in the implementation of the medical care information provision service through the Internet, which is the key business of CareNet, and hereby establishes the basic policy for the achievement of the following purposes and ensures that all employees of CareNet understand and comply with this policy.
1. Collection, Utilization and Provision of Personal Information
CareNet shall, in collecting personal information, clarify the purpose and use appropriate and fair means to the extent necessary for the achievement of the purpose. CareNet shall, in utilizing and providing personal information, conduct the utilization and provision of personal information in compliance with laws and regulations and within a scope clarified in advance, and shall take measures to avoid utilization of personal information for unintended purposes.
2. Response to Request for Disclosure, Correction, etc.
When CareNet receives a disclosure request from a principle as to the personal information retained by CareNet and for which CareNet has the authority to make disclosure, etc. (hereinafter referred to as “Retained Personal Data”), the Company shall respond to such request within a reasonable period and within an appropriate scope. When there is an error or a change in personal information and the principle pertaining to such personal information makes a request for correction, etc., CareNet shall respond to such request within a reasonable period and within a necessary scope.
3. Implementation of Security Measures
CareNet shall, in order to ensure the correctness and security of personal information, implement information security measures and other security measures, and securely prevent the loss, destruction, alteration, leakage etc. of personal information. When CareNet’s security measures need improvement as a result of an internal audit or based on actual cases of security accidents or at the request of customers or employees of CareNet, CareNet shall promptly act to improve its security measures.
4. Compliance with Laws, Regulations, Codes
CareNet shall comply with the applicable laws and regulations and the guidelines and codes stipulated by the State with respect to the handling of personal information. CareNet shall ensure that the CareNet Personal Information Protection Management System conforms to these laws and regulations and to the guidelines and codes stipulated by the State.
5. Continuous Improvement of Personal Information Protection and Management
CareNet shall, in order to maintain the effect of the Personal Information Protection Management System established by CareNet for personal information, continuously improve and maintain in a good condition the Personal Information Protection Management System on the occasion of regular internal audits by CareNet and reviews of the Personal Information Protection Management System by the President and COO.
6. Complaint and Consultation on Personal Information
CareNet shall appropriately respond to, and steadily take corrective measures for, complaints and consultation from customers about CareNet’s handling and management system of personal information provided by customers and kept by CareNet.

Personal Information Protection Regulations
1. Personal Information Collected
CareNet may collect the following items of information to provide medical care information, deliver goods and make confirmation and request for payment. CareNet may ask for a Login ID to confirm the identity of a principle.

  • • Gender
  • • Subject of medical treatment
  • • Information on employment place
  • • Email address
  • • Telephone number and fax number
  • • Credit card information
  • • Shareholder information
2. Purpose of Utilization of Personal Information
CareNet shall utilize your personal information for the following purposes:
  • • Provision of medical information
  • • Survey by questionnaire
  • • Response to request for materials concerning goods and services of CareNet
  • • Request for payment of charges for use of onerous services
  • • Processing of delivery of purchased goods

Also CareNet shall utilize personal information of shareholders for the following purposes:
  • • Fulfillment of obligation to exercise shareholders' rights under the Company Law etc.
  • • Providing various kinds of convenience for the position as a shareholder
  • • Management of shareholders based on laws such as the Company Law
  • • Implementation of various shareholder policies
3. Provision of Personal Information to a Third Party
  • (1)CareNet shall not provide your personal information to a third party except for cases in which your prior consent is obtained, cases based on laws and regulations and cases in which personal information is entrusted within a necessary scope.
  • (2)When CareNet entrusts your personal information, CareNet shall ensure that the entrustee is restricted to a company confirmed by CareNet as having an organization that complies with the “Personal Information Protection Act” (Act No. 57 of 2003) and other related laws and regulations and also the “Guideline Concerning the Personal Information Protection Act” (hereinafter referred to collectively as “Personal Information Protection Act, etc.”) and that takes organizational, human, physical and technical security control measures. Furthermore, CareNet shall conclude with such entrustee a contract concerning confidentiality obligation and personal information protection obligation in advance.
4. Joint Utilization
CareNet shall utilize the medical data base managed by Nihon Ultmarc Inc. (hereinafter abbreviated as “MDB”) jointly with the member companies consisting mainly of pharmaceutical companies in the fields restricted to medical care, welfare, health, etc. (hereinafter referred to as “Member Companies”). As to the purpose of utilization, etc. of MDB, please refer to the website of Nihon Ultmarc Inc. (http://www.ultmarc.co.jp/contents/PrivacyPolicy.html).

  • Among personal information pertaining to members, information on doctor name, classification of practitioners and working doctors, year of commencement of practice, gender, place of employment, subject of medical treatment, year of acquisition of medical license, university from which a member graduated from, date of birth and medical association in which a member enrolls

  • MDB shall be utilized not only for the collection and review of necessary information for the provision of the services on the website “CareNet.com” managed by CareNet (hereinafter referred to as “CareNet.com”) and each Internet service incidental thereto but also for the provision of information to members of such services.

5. Anonymously Processed Information
When CareNet produces information relating to an individual that can be produced from processing personal information so as neither to be able to identify a specific individual nor to be able to restore the personal information (hereinafter referred to as “anonymously processed information”), CareNet shall announce to the public the items pertaining to the individual that are contained in such anonymously processed information in accordance with the Personal Information Protection Act, etc. Moreover, CareNet shall comply with the standards prescribed in the Personal Information Protection Act, etc. when producing anonymously processed information, and CareNet shall, when having produced anonymously processed information, not collate the said anonymously processed information with other information in order to identify a principal concerned with personal information used to produce the said anonymously processed information. Furthermore, CareNet shall, when providing anonymously processed information produced by CareNet to a third party, make public the items pertaining to the individual that are contained in said anonymously processed information and the method of provision, and clearly indicate to said third party that information provided is anonymously processed information in accordance with the Personal Information Protection Act, etc.
6. Utilization of IP Address and Cookies
CareNet shall utilize, mainly for the following purposes, the IP addresses of all users recorded in the access logs on the websites managed and operated by CareNet:
  • • To pinpoint the cause for and solve a problem that may occur in the server
  • • To manage the websites

In addition, CareNet may use cookies. Cookies are small data files transmitted between a website server and the user’s web browser and may be stored in the user’s disk as a file. By using cookies, the websites can keep records of what pages are visited by users. CareNet shall use cookies for the following purposes:
  • • to enable a user to avoid having to input the password each time the user accesses the website
  • • to provide medical care information and advertising information that meet the concerns of all users

CareNet may, when having entrusted its business to a third party for the provision of advertisement, use cookies via such entrustee. CareNet shall properly select and supervise such entrustee in accordance with the relevant rules of CareNet.
Furthermore, CareNet may utilize advertisement distribution services provided by third parties. In such cases, said third parties may acquire and use cookies. Cookies acquired by said third parties shall be handled in accordance with the privacy policies of said third parties.
If you wish to inactivate the use of cookies concerning the provision of advertisement, you may access the opt-out page on each website where an advertisement is displayed and inactivate the use of cookies in accordance with the relevant procedure. CareNet shall, on receiving from you a request to notify the purpose of use of, disclose, correct, add or delete the contents of, cease use of, erase, and cease third-party provision of, retained personal data (such actions shall be hereinafter referred to as “disclosure, etc.”), without delay conduct an investigation and properly respond to such request for disclosure, etc.
