CareNet, Inc., (hereinafter referred to as “CareNet”) recognizes the importance of managing the personal information of customers when providing medical information services via the Internet, which is our core business, in addition to managing the personal information of employees safely; therefore, CareNet establishes this basic policy to achieve the following objectives and will keep all employees fully informed of any changes.
1. Collection, Use and Provision of Personal Information
Personal information shall be collected after specifying the purpose while using legitimate and fair means to the extent necessary to achieve the purpose. The use and provision of personal information shall be compliant with all laws and regulations and within the range of the purpose of use specified in advance. CareNet shall take the appropriate precautions to prevent the use of personal information for purposes other than the intended purpose.
2. Response to Request for Disclosure or Amendment
If CareNet receives a request from a customer to disclose personal information, CareNet shall respond to such request within a reasonable period of time and within the appropriate range. If there is any error or change in personal information and the relevant customer requests an amendment to such information, CareNet shall respond to such request within a reasonable period of time and to the extent necessary.
3. Implementation of Safety Precautions
To ensure the accuracy and security of personal information, CareNet shall implement a variety of safety precautions, such as information security measures, to prevent the unauthorized access to personal information or the loss, destruction, tampering, or leakage of personal information. If any improvement is required as a result of internal audits, actual security incidents, requests from customers or from within the company, or any other reasons, CareNet shall promptly take the action necessary to make the improvement.
4. Compliance with Laws, Regulations, and Standards
CareNet shall comply with all laws, regulations, national government guidelines, and other standards that apply to the protection and handling of personal information. CareNet shall make sure that the personal information protection management system of CareNet meets the requirements of these laws, regulations, national government guidelines, and other standards.
5. Continuous Improvement of Protection and Management of Personal Information
To maintain the effectiveness of the personal information protection management system formulated by the company, CareNet shall continuously improve the personal information protection management system through periodic reviews of the personal information protection management system conducted by the internal auditors and the president and CEO to maintain the system in the best state at all times.
6. Complaints or Inquiries Concerning Personal Information
CareNet shall appropriately respond to complaints or inquiries received from customers concerning the handling and management system for personal information provided by customers and shall ensure that CareNet will take corrective actions.

Regulations for The Protection of Personal information
1. Collection of personal information
CareNet may collect the following personal information from customers through this website.
  • • Name
  • • Address
  • • Date of birth
  • • Gender
  • • Information on your workplace
  • • E-mail address
  • • Phone number and fax number
  • • Matters related to shares held
2. Purpose for using personal information
CareNet will use personal information for the following purposes.
  • • Responses to inquiries and for communication
  • • Responses to requests for brochures of CareNet’s products and services
  • • Shareholder benefits
3. Provision of personal information to third parties
CareNet will not provide personal information to any third party except as follows: when CareNet obtains prior consent from a customer, as required by law or regulations, or when CareNet entrusts personal information to a third party to the extent required. When CareNet entrusts personal information, the contractor shall be limited to any corporation for which CareNet has confirmed that such contractor complies with the Act on the Protection of Personal Information (Act No.57 of 2003), other relevant laws and regulations, and the Guidelines for the Act on the Protection of Personal Information in Economic and Industrial Fields, and that such contractor is an organization taking security control precautions from the organizational, personnel, physical, and technical perspectives. In addition, CareNet shall enter into agreements with the contractor in advance related to the obligations of confidentiality and the protection of personal information.
4. Responses to inquiries concerning personal information collected and other matters
When CareNet receives inquiries or complaints regarding the handling of personal information collected from customers or receives requests for disclosure or other handling (notification of the purpose of use; disclosure, amendment, addition, or deletion of contents; or the suspension of use, elimination, and cessation of supply to third parties, which are collectively referred to as requests), CareNet shall conduct an investigation without delay and respond to such requests appropriately.

  • A handling fee of five hundred (500) yen per request shall be charged in response to the following requests.

  • No handling fee will be charged for amendment and suspension of use.

  • After requesting disclosure or other handling, a customer is required to submit a copy of any of the following documents by mail. When mailing the documents, please enclose postage stamps or a postal money order with a fixed amount equivalent to five hundred yen (500 yen).

  • • Health insurance card
  • • Pension handbook
  • • Certificate of residence
  • • Passport
  • • Certificate of alien registration
  • (When a representative requests disclosure or other handling)
  • In addition to copies of any of the documents referred to in the preceding item, the customer must submit a document via mail that can verify that the person is your representative. When mailing, enclose postage stamps or a postal money order with a fixed amount equivalent to five hundred (500) yen.

  • As for a request for disclosure or other handling received from a customer, CareNet will respond to such request as soon as possible. However, the response may be delayed for certain reasons. In the following cases, CareNet may not respond to the request for disclosure or other handling. In such case, CareNet will notify the customer to that effect and the reason.
  • • When an individual cannot be confirmed
  • • When a representative cannot be confirmed in cases where a request was made by the representative
  • • When there is a deficiency in the document submitted for the request
  • • When the information subject to the request for disclosure or other handling is not personal information subject to disclosure
  • • When there is a risk of harm to the life, body, property, or other rights or interests of the individual or third parties
  • • When there is a risk of significant obstruction to the business operations of CareNet
  • • When disclosure constitutes a violation of other laws and regulations
5. Handling of personal information of employees, etc.
